Immigration Evaluations:

Dr. Caspe is a designated Civil Surgeon that performs immigration evaluations.  The evaluation includes  a physical examination, tuberculosis test and RPR (syhphilis test).  The exam involves a complete history and physical examination including examination of the genitalia.   Please call the office for current price.

Please bring the following when you come in for an immigration evaluation:

1.  Valid photo ID ( ex passport, drivers license)

2. Immunization records

3. Medical records that contain information on any current medical or psychiatric condition.

If you need immunizations we refer you to a local pharmacy for the vaccines.  Immunizations are determined based on age of the person seeking the immigartion evaluation.
